Registration

Online Registration at this website is the most preferred way.  Participants and accompanying persons are requested to register in advance by completing and submitting the Registration Form with appropriate fees. Registration is also possible by fax or regular mail. In order to avoid double charges, please submit your registration ONCE.

ACCOMMODATION AND SHUTTLE BUS
Please note that the shuttle bus from the distant hotels to Venue will be provided only to the
participants who have booked their accommodation through the PCO agency two times a day.

Morning shuttle bus: before beginning of the sessions.
Afternoon shuttle bus: after the programme.
Please check the web site at the later time for detail timetable of shuttle buses.

Fellowships

The Local Organising Committee of the 7th EADV Spring Symposium will offer 50 grants of reduced registration fee. The fee reduction will be €100/case for the applicants whose economic means are not otherwise supported to attend the above mentioned Symposium, 13-16 May, 2010, Cavtat, Croatia and have not already benefited from this offer in the past. Fellowships are only assigned to individuals. Companies or travel agencies are not eligible for fellowships. Please note that block bookings receive no consideration. If you would like to apply for a grant, please send a copy of your CV and a covering letter to the address below. Please make clear in your covering letter your reasons for wishing to attend the meeting.

Name Badge

All delegates and accompanying persons will receive a name badge on-site. The name badge is your ticket for booked activities and sessions. Admission to the Symposium will not be allowed without badge identification.

 

Confirmation

When appropriate fees have been received, a confirmation/invoice will be sent to participants. This authentification should be presented at the registration desk in Cavtat.

 

Cancellation Policy

REGISTRATION
Cancellation of participation made before 14 January, 2010
Paid registration fee will be refunded deducting 25 EUR of handling fee per person.
After that, date there will be no refund whatsoever.

ACCOMMODATION
Cancellation made before 8 February, 2010
Full refund of the accommodation fee will be made deducting 30 EUR of handling fee.

Cancellation made between 8 February - 8 March, 2010
One night accommodation fee will be charged along with 30 EUR of handling fee.

Cancellation made after 8 March, 2010
No refund will be made.

Name changes will be charged with 20 EUR per registration. Should you have any special request for the processing of your invoice, kindly let us know when returning your registration form. For the rewriting of the invoice 20 EUR will be charged. All refunds will be made within 2 months after the end of the Symposium.
Bank charges might be deducted.

These general conditions and all administrative information are to be considered as the legal basis for all reservations. Verbal agreement are not binding, unless confirmed in writing.

Letter of Invitation

Those persons needing an official Invitation Letter in order to attend the Symposium may contact: info@eadvcavtat2010.com specifying the necessary details.
This service is designed only to assist participants who need to obtain a visa or permission to attend the Symposium. It should not be considered as an official invitation covering fees or other expenses

 

Certificate of Attendance

All registered attendees both pre-registered and on-site will receive a certificate of attendance issued on-site.

 

Rules - Presenters at EADV Academic meetings

PATIENT PRIVACY/ CLINICAL PHOTOGRAPHS / CONFLICT OF INTEREST

PHOTOGRAPHS should at all times protect the privacy and identity of the subject. 

Eyes should be blacked out.
The source of the photographs must also be declared if not property of the presenter.
This is to stop use of photographic material without permission.

Conflict of Interest
A signed Declaration of Interest form is required from all presenters at EADV academic meetings.

It should include:
1. Paid work by Commercial Organisations either directly or indirectly through an intermediary.
2. Receipt / ownership of shares in Commercial Companies.
3. Receipt of travelling, accommodation and hospitality expenses from Commercial organisations.

Registration, Information & CME Desk

The registration desk will be located in the entrance hall of the Symposium venue: Hotel Croatia, Cavtat.
The CME desk as well as the information desk will be located close to the registration desk.

 

Exhibition

A large industry exhibition will run during the Symposia. Interested companies are advised
to contact the PCO at: exhibition@eadvcavtat2010.com.

 

Symposium Venue

HOTEL CROATIA  is the leading five star hotel on the Croatian coast. It is located in the picturesque bay of the little town of Cavtat, which is a part of the Dubrovnik Riviera, world–known by the beauty of its Mediterranean landscapes and clear, unpolluted sea. The famous medieval walled city of Dubrovnik can be seen from some hotel terraces. It would be difficult to find similar hotel, where the architect has so cleverly succeeded to incorporate all amenities of a modern five–star hotel into the surrounding green vegetation and the beautiful blue colors of the Adriatic sea.

LOCATION: Hotel Croatia is located in Cavtat at the very south of the Republic of Croatia; 18 km from Dubrovnik and only 6 km from Dubrovnik International Airport Ćilipi.
